Ice Wolves 2011-2012 Player Registration Instructions

Our umbrella organizations, the Wisconsin Amateur Hockey Association (WAHA) and USA Hockey have adopted online registration that requires parents to register each skater’s name and address for the 2011-2012 season and pay a registration fee directly to USA Hockey. This is in addition to completing the Ice Wolves Registration forms and registration fees.

For paying these fees, USA Hockey members receive medical, catastrophic and liability insurance coverage, a membership number and a subscription to USA Hockey Magazine. The USA Hockey registration portion is quite simple and should only take a few minutes to complete. Registration instructions are provided below.

To enroll your skater(s) for the 2011-2012 season:

1. Register online with USA Hockey and obtain a confirmation number for EACH SKATER:

Follow the prompts…fill in all data correctly. Payment is made using a credit card-Visa, Mastercard, American Express or Discover Card.

When you complete your USA Hockey online registration, you will receive an email with your conformation number and separate barcode for each skater.

Please forward this email to: Wendy Biddick or print it off and send it in with your IWYHA registration form. Please be sure the copy contains the barcode.

2. Complete the following forms (as skater / player)

3. Mail or hand in the above forms and the USA Hockey confirmation number and pay IWYHA registration fees by deadline (see Skater Registration Form)
